Passed away peacefully on Monday, February 14, 2022 in her 89th year. Beloved wife of the late Bill Formanek. Cherished mother of Richard and Robert (Sherry). Loving sister of Mary Ann Knotek (late George) and the late Gerry Authier (John). Dear aunt to many nieces and nephews. Predeceased by her parents John and Marie Chabica, formerly of Chatham. The family would like to thank the staff at Windermere on the Mount, peopleCare Oakcrossing LTC and the London Regional Cancer Centre for their kindness and care. Cremation has taken place. A memorial service will be conducted at Westview Funeral Chapel, 709 Wonderland Road North, London on Friday, February 18, 2022 at 1:00 p.m., with visitation from 12:00-1:00 p.m. Registration will be required to attend the visitation and service. Interment to follow at St. Peter’s Cemetery.
